*Terms of Membership: By the use of the facilities of Wimbledon Management Co. d.b.a. Wimbledon Racquet Club and/or by the attendance at any of the activities of Wimbledon Management Co. d.b.a. Wimbledon Racquet Club, the Member expressly agrees that Wimbledon Management Co. d.b.a. Wimbledon Racquet Club shall not be liable for any damages arising from personal injuries sustained by the member or his/her guest in, on or about the premises or as a result of their using the facilities and the equipment therin. By the execution of this agreement Member assumes full responsibility of any such injuries or damages which may occur to the Member or guest in, on or about the premises of Wimbledon Management Co. d.b.a. Wimbledon Racquet Club and further agrees that Wimbledon Management Co. d.b.a. Wimbledon Racquet Club shall not be liable for any loss or theft of personal property. Member assures full responsibility for any injuries, damadges or losses which may occur to Member or guest in, on or about the premises and does hereby fully and forever release and discharge Wimbledon Management Co. d.b.a. Wimbledon Racquet Club, their owners, employees and agents from any and all claims, demands, damages, rights of action, or causes of action, present or future, whether the same be known or unkown, anticipated or unanticipated, resulting from or arising out of the Member's or his/her guest's use or intended use of Wimbledon Management Co. d.b.a. Wimbledon Racquet Club or the facilities and equipment thereof.

RETAIL CREDIT AGREEMENT

The cash price of goods and services purchased from Wimbledon Racquet Club is due on the last day of the month, otherwise a FINANCE CHARGE (Time price differential) will be computed on the UNPAID BALANCE. To avoid FINANCE CHARGE pay full new balance by the last day of the month. The monthly finance charge of 1 1/2 per month or 18% ANNUALLY will be calculated on the AVERAGE DAILY BALANCE. A $25.00 fee will be added to your account for checks returned for insufficient funds.

For your convenience, membership will automatically be renewed 1 year from your enrollment date, unless we are notified otherwise.
